Cutaneous leishmaniasis is a skin infection caused by Leishmania parasites, which are spread to humans through the bite from an infected sandfly. Though more prevalent in regions of the Middle East, South America, Africa, and Asia, world travel and military deployment bring the cases closer to home, so awareness and prompt diagnosis are important.

What Causes Cutaneous Leishmanias

The most common reason for cutaneous leishmaniasis is the bite of an infected female phlebotomine sandfly that carries the Leishmania parasite. Sandflies live and reproduce best under the heat and warmth of the tropics and subtropics and are usually most active between dusk and dawn.

Individuals who are at risk include international travellers, military personnel, and people spending significant amounts of time outdoors in endemic areas. Poor housing and deforestation can also raise the risk for exposure to sandflies.

Recognizing the Symptoms

Cutaneous leishmaniasis may start as a small, red bump that can be mistaken for a bug bite. As it develops, the lesion may turn into an open ulcer or sore with raised borders and a central crater. It can be painless or mildly painful and most often develop on areas of the skin that are not covered, like the face, arms, and legs.

Other indications to observe include:

  • Wounds or skin lesions which do not heal within weeks or months

  • Scabbing or crusting of the ulcer

  • Swollen lymph nodes over the affected region

These sores, if left untreated, can create scarring, secondary infections, and long-term cosmetic issues, especially when they develop on the exposed parts of the face.
